Kuleana Program Waiver & Release of Liability Form

In order for the participant to participate in the full day Kuleana: Waste Reduction Program on July 7th in Hawaii, the following Waiver & Release of Liability Form must be thoroughly read and signed accordingly.

The form may be signed in three different ways.

​

Option 1 - Print out the form, sign, and turn it in physically and directly to an Abbey Road Travel staff member prior to July 7th.

Option 2 - Print out the form, sign, scan/take a clear photo of the form, and upload it in the form below. 

Option 3 - Fill out the form and digitally sign below.

The deadline to turn this form in is no later than prior to your departure from Hawaii.

3.  If the participant is under the age of 18, type in parent/guardian's name below.

4.  By signing below digitally, I certify that I have read the attached waiver form titled "Kuleana: Sustainable Hawaii Education Program; July 7, 2021 - Accident Waiver And Release of Liability Form," and I fully understand its content. I am aware that this is a release of liability and a contract and I sign it of my own free will.
